Global Surgery Fellowship (must have completed an ACGME approved general surgery residency) Loma Linda University Medical Center Date Posted: November 22, 2017The Loma Linda University Department of Surgery in Loma Linda, California has begun a new one year non-ACGME accredited Global Surgery Fellowship (GSF) and is looking to find its next fellow to start August 1, 2018. The goal of the Global Surgery Fellowship is to prepare American-trained general surgeons to be competent and capable of performing the needed surgeries in low and middle income countries (LMIC). The Global Surgery Fellowship furthers the Loma Linda University Health (LLUH) goal ‘to make man whole’ physically, intellectually, emotionally, and spiritually by continuing Christ’s healing and teaching ministry.

It aims to address the overwhelming surgical needs throughout the developing world. In 2015, the World Health Organization recognized that the lack of access to safe surgical care results in more deaths than HIV, TB, and Malaria combined and the WHO made Global Surgery a priority in public health.

There are approximately five billion people around the globe who lack access to safe and affordable surgical and anesthesia care with a vast majority of those people living in the low-resource settings of low and middle income countries. It is estimated that each year 143 million additional surgical procedures need to be done in Sub-Saharan Africa alone. The LLUH GSF will directly address surgery’s indispensable part of health care in underserved areas and will positively affect the U.N. Sustainable Development Goals (SDGs).

The Global Surgery Fellowship (GSF) is a one year training program for American Board of Surgery eligible or certified general surgeons. It consists of nine months training at Loma Linda University Health (LLUH) in Loma Linda, California and 3 months of guidance and experience at Malamulo Adventist Hospital (MAH) in Malawi, Africa. The fellowship training at LLUH will consist of rotations and case log completion in Obstetrics, Gynecology, Urology, Orthopedics, and Trauma, as well as Anesthesia, Neurosurgery, Pediatric Surgery, ENT, Plastic Surgery, Wound Care, Dentistry, Preventative Medicine, Tropical Disease, Public Health, financial literacy for organizations, and leadership in health administration. The fellow will be eligible to take the exam for the Diploma of the American Society of Tropical Medicine and Hygiene after the completion of the required modules and in-person session. The fellow will complete at least one research paper focusing on a topic in Global Surgery. LLUH is uniquely equipped for the GSF with its strong history of international service and its direct connection to a wide network of hospitals in low and middle income countries. This modus operandi of mission focus is evident in the multidisciplinary enthusiasm and support for the Department of Surgery’s Global Surgery Fellowship.

The fellowship program director, Dr. Carlos Garberoglio, and assistant program director, Dr. Ryan Hayton, have a continued interest in meeting the global needs of surgery based on their real experience in providing surgical care in low-resource environments. Applicants must have completed an ACGME approved general surgery residency by June 30, 2018 and be American Board of Surgery certified or eligible.

Since 1997, the American College of Surgeons has recognized BUMC as a Level 1 Trauma Center with approximately 3,400 patients evaluated and 2,400 admissions yearly.

Penetrating injury accounts for approximately 17% of our trauma patients and we expect the fellows to operate and improve upon their surgical skills. There are 123 total ICU beds, 57 of which are surgical ICU beds. Currently, our surgical critical care faculty provides care for trauma, acute care surgery, general surgery, solid organ transplant, heart and lung transplant, cardiothoracic, neurosurgery, orthopedic and orthopedic spinal surgery, ENT, oral surgery, OB-GYN, and plastic surgery. The surgeons in the Division of Trauma, Critical Care and Acute Care Surgery have always been closely involved in general surgery resident education (9 residents a year), as well as research and quality initiatives. Our surgeons also serve as Texas A & M Health Science Center College of Medicine faculty and are heavily involved in medical student education. There are 48 medical students in each class that rotate at our campus starting from their 2nd year onwards. The Division of Trauma, Critical Care and Acute Care Surgery continues to grow and currently includes 13 faculty physicians, 12 of whom are board certified with Added Qualifications in Surgical Critical Care.

The ACGME approved our new Surgical Critical Care Fellowship in March 2016 for four (4) fellowship positions. We have one spot open for the 2018-2019 academic year.

The goal of our Surgical Critical Care Fellowship program is to prepare the fellows to function as qualified practitioners at the advanced level of performance expected of Board-certified subspecialists. The fellowship will allow fellows to develop proficiency in the management of critically-ill and injured patients, develop necessary qualifications to supervise surgical critical care units, and to conduct scholarly activities in trauma/surgical critical care. The 12-month fellowship is designed to provide surgical critical care fellows with an education in the principles and practice of state-of-the art trauma and surgical critical care according to medical knowledge and patient care, practice-based learning and improvement, systems-based practice, professionalism, and interpersonal and communication skills.

This is accomplished by exposing the surgical critical care fellows to a broad array of surgical illnesses through didactic instruction in the basic and clinical sciences, as well as education during teaching rounds, primary patient care, educational conferences, and specialized rotations. Upon completion of training, the surgical critical care fellows are expected to integrate the acquired knowledge into the clinical situation to demonstrate proficiency in surgical critical care decision-making, specific organ system support, evaluation of new technology and treatment techniques, ICU administration, outcomes assessment, research design, and interaction with patients, families, and health care personnel. The Surgical Critical Care Fellowship will be individualized to each applicant and is focused on developing their future practices whether in academics or a private setting. The fellowship year is also customized to address specific deficits that the candidate feels that he/she might have with respect to operative, non-operative, and interventional management. Housestaff physicians are selected from among eligible applicants on a compilation of the following: preparedness, ability, aptitude, academic credentials, communication skills and personal qualities such as motivation and integrity. For Surgical Critical Care Fellowship, Surgical critical care and Acute care surgery, we use the Surgical critical care and Acute care surgery Fellowship Application Service (SAFAS). Candidates are required to submit an application accompanied by a Dean's letter, a transcript, three letters of recommendation from attending physicians and a personal statement. A personal interview is required. The Selection Committee evaluates each applicant concerning aptitude, academic credentials, characteristics and ability to communicate.

Global Surgery Fellowship Position (Non-ACGME) Loma Linda University Medical Center Date Posted: August 14, 2017The Loma Linda University Department of Surgery in Loma Linda, California has begun a new one year non-ACGME accredited Global Surgery Fellowship (GSF) and is looking to find its next fellow to start August 1, 2018.

International Surgical Oncology Global Cancer Disparities Fellowship Memorial Sloan Kettering Date Posted: August 3, 2017The Department of Surgery offers a one-year International Surgical Oncology Global Cancer Disparities Fellowship to fully trained surgeons.

The aim of the International Surgical Oncology Global Cancer Disparities Fellowship is to train the fellow to effectively direct multimodality cancer therapy for the cancer patients in both high- and low-income countries. Cancer is a rapidly growing problem in low- and middle-income (LMIC) countries. Outcomes for cancer patients are poor. There is a need for improved research and training to improve these outcomes. The MSK Global Cancer Disparity Initiative was created in 2013. The goal of the initiative is to improve outcomes for patients with cancer in LMIC. It is based on a collaboration between physicians at MSK and Nigeria that began in 2010.

This collaboration has now grown to include the African Research Group for Oncology (ARGO) in Nigeria, which is an MSK cofounded consortium that includes 5 centers in Nigeria and MSK. ARGO has multiple ongoing training endeavors and clinical trials regarding colorectal and breast cancer. In addition, in 2016, a new collaboration was created with the Aga Khan hospitals in Kenya and Tanzania, with a focus on breast and cervical cancer training and research. CLINICAL TRAINING The training throughout the fellowship is comprised of a rotation schedule. Fellows will spend 3 months in breast and colorectal services at MSK, 3 months on clinical research/elective time at MSK, and 6 months at MSK’s collaborating sites in West and East Africa.

While they are on clinical services, fellows are responsible for all preoperative, intraoperative and postoperative patient care. In addition, they are responsible for teaching residents and medical students who are rotating on the service. While in research months at MSK, fellows will attend colorectal and breast cancer clinical conferences while performing research in global cancer disparity topics. Emphasis is placed on understanding and developing multimodality treatment protocols for the range of malignancies which are encountered by surgical oncologists in practice in both high- and low-income countries. Trainees are expected to gain a thorough understanding of cancer biology, cancer disparities, cancer care in resource limited environments, genetics, biostatistics and epidemiology.

This is done through lectures, conferences, grant applications, directed readings and individual research projects. RESEARCH Fellows will become well-versed in the current literature with a specific reading list provided by the global cancer disparity fellowship. The fellow will have a dedicated mentor on the Colorectal Service and the Breast Service to guide research projects. In addition, the Global Disparities Director will have weekly meetings to assist with research projects and provide weekly education sessions specific to global cancer initiatives. Education during these 3 months will include Dept.

Of Surgery conferences such as Grand Rounds, Surgical Oncology Fellows Teaching Conference, the Gastric Mixed Tumor, Breast, and Colorectal Service Disease Management Team conferences. The global disparities fellow will participate in the surgery fellow research training course. This course, over an 8-week period, includes training in topics such as database management, research ethics, clinical trial design, and statistics.

Over the course of the year, the fellow will create: Two retrospective research projects answering 2 important clinical research topics related to cancer in low- and middle income countries (LMIC). These projects will utilize the MSK supported databases and biobanks in Nigeria with breast and colorectal cancer patients. One prospective study that will be initiated during their clinical/research rotation in Nigeria. This study will focus on topics such as barriers to cancer care, implementation of treatment pathways, and methods for the early detection of cancer. The fellow will also inherit the prospective study that was created by the prior fellow – thus each fellow will generate a prospective study they will develop and initiate and inherit an already established prospective protocol that they will actively manage. Assist with writing one grant application specific to cancer research or training in LMIC.

This will provide the fellow with valuable experience in synthesizing LMIC cancer topics, creating specific aims, grant structure, and the logistics of creating a grant application. Send one abstract to a conference with a topic specific to cancer in LMIC. Global Surgery Fellowship Position Loma Linda University Medical Center Date Posted: April 7, 2017Dear Program Director: The Loma Linda University Health Department of Surgery in Loma Linda, California is beginning a new one year non-ACGME accredited Global Surgery Fellowship (GSF) starting August 1, 2017. The goal of the Global Surgery Fellowship is to prepare Christian, American-trained general surgeons to be competent and capable of performing the needed surgeries in low and middle income countries (LMIC). The Global Surgery Fellowship furthers the Loma Linda University Health goal ‘to make man whole’ physically, intellectually, emotionally, and spiritually by continuing Christ’s healing and teaching ministry.

The Global Surgery Fellowship (GSF) is a one year training for American Board of Surgery eligible or certified general surgeons. It consists of nine months training at Loma Linda University Health (LLUH) in Loma Linda, California and 3 months of guidance and experience at Malamulo Adventist Hospital (MAH) in Malawi, Africa. The fellowship training at LLUH will consist of rotations and case log completion in Obstetrics, Gynecology, Urology, Orthopedics, and Trauma, as well as Anesthesia, Neurosurgery, Pediatric Surgery, ENT, Plastic Surgery, Wound Care, Dentistry, Preventative Medicine, Infections Disease, Tropical Disease, Public Health, Financial Literacy for organizations, and Leadership in Health Administration. The GSF will include a Diploma of the American Society of Tropical Medicine and Hygiene as well as a membership in the Consortium of Universities for Global Health.

Applicants must have completed an ACGME approved general surgery residency by June 30, 2017 and be American Board of Surgery certified or eligible.
